Job summary

Infinite Medical P.C. is seeking a dedicated Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within our Collaborative Care Model program.

This is an in-office, non-face-to-face care management position that remotely supports residents in skilled nursing and long-term care facilities through medical record review, behavioral health screening, care planning, registry management, and team collaboration.

Qualifications

• Licensed Practical Nurse (LPN) license.
• Experience in behavioral health within a collaborative care model (CoCM).
• Ability to perform non-face-to-face care management in skilled nursing or long-term care settings.

Responsibilities

• Review patient medical records, diagnoses, medications, behavioral health documentation, provider notes, labs, and changes.
• Document validated behavioral health screening and monitoring tools (PHQ-9, GAD-7, MoCA, BIMS).
• Monitor behavioral health symptoms, treatment response, functional status, and progress toward goals.
• Identify changes requiring escalation and safety concerns.
• Maintain ongoing non-face-to-face contact with patients, representatives, facility staff, and care team.
• Develop and maintain individualized behavioral health care plans with the care team.
