Job summary

Infinite Medical P.C. is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within a Collaborative Care Model. This is an in-office, non-face-to-face role supporting residents in skilled nursing and long-term care facilities.

You will review records, document screenings (PHQ-9, GAD-7, MoCA, BIMS), develop care plans, coordinate services, and collaborate with a multidisciplinary team to improve outcomes in senior care.

Qualifications

• Active, unrestricted LPN license and grad of an accredited practical nursing program.
• Strong clinical documentation and record-review skills.
• Ability to elevate behavioral health and patient-safety concerns when needed.

Responsibilities

• Review patient records, diagnoses, meds, and behavioral health data.
• Document PHQ-9, GAD-7, MoCA, BIMS and other assessments.
• Maintain non-face-to-face contact with patients and care teams.
• Develop and update individualized behavioral health care plans.
• Coordinate behavioral health services and follow-up recommendations.
• Prepare patient case summaries for psychiatric consultations.
