Infinite Medical P.C. is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within our Collaborative Care Model (CoCM) program.

This is an in-office, non-face-to-face care management position supporting residents in skilled nursing facilities through chart review, screening, and care coordination. The role involves working with an integrated care team, developing individualized care plans, and coordinating behavioral health services while adhering to HIPAA and

Qualifications

• Active LPN license required.
• Graduate of an accredited practical nursing program.
• Strong clinical documentation skills.
• Ability to collaborate with providers and care teams.
• HIPAA compliance and patient confidentiality.

Responsibilities

• Review patient records and behavioral health data.
• Develop and update behavioral health care plans.
• Coordinate services and follow-up per CoCM guidelines.
• Document and escalate clinical findings as needed.
