Job summary

Trilogy Health Services in Indiana is seeking an Assisted Living Coordinator to support health services for residents. You will work with the DON/ADON, manage assessments, care plans, ADLs, and medications, and lead a care team.

This role requires an active LPN license, CPR certification, and leadership experience; experience in assisted living is preferred. We offer comprehensive Thrive benefits and weekly pay.

Responsibilities

• Collaborates with the DON/ADON on day-to-day clinical operations for the campus.
• Ensures pre-admission screenings are completed prior to move-in.
• Directs resident assessments to determine medical and personal care needs.
• Assists residents with ADLs and medication administration.
• Leads development of service plans with resident/family and communicates plans to caregivers.
• Facilitates Assisted Living Resident Meetings and follows up on feedback.
• Serves as lead nurse for assisted living residents and participates in on-call rotation.
