Job summary

Trilogy Health Services, LLC is seeking an Assisted Living Coordinator in Columbus, IN to oversee health assessments, medication management, and daily living activities for residents.

You will collaborate with the Director of Nursing and Assistant Director of Nursing to ensure smooth clinical operations, lead service plan development with families, and participate in on-call rotations. An active LPN license and CPR are required; leadership experience is preferred.

Qualifications

• Active LPN license in the state
• CPR certification required
• Previous assisted living and leadership experience preferred

Responsibilities

• Collaborates with the DON and ADON for campus clinical operations.
• Ensures pre-admission screens are completed prior to move-in.
• Directs lifestyle and care needs assessments to determine resident needs.
• Assists residents with activities of daily living and medication administration.
• Develops service plans with resident/family and ensures caregivers are informed.
