Job summary

PACS Group is seeking an LPN for all shifts at Evan Terrace Post Acute in Oregon. The role provides direct patient care, medication administration, vital signs monitoring, and collaboration with the nursing team to maintain high-quality care in a Skilled Nursing Facility.

The ideal candidate holds an LPN license with CPR/First Aid certification, and experience in long-term care is preferred. Excellent communication, compassion, and teamwork are essential in this fast-paced setting.

Qualifications

• Licensed Practical Nurse (LPN) licensure in state of practice.
• CPR and First Aid certification required (or obtainable on hire).
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Provide direct patient care and administer medications as prescribed.
• Monitor residents' vital signs and document changes.
• Assist with daily living activities and mobility tasks.
• Collaborate with nursing team to implement care plans and ensure quality care.
• Document accurately in resident records and report to DON or Charge Nurse.
