Job summary

Cascade Terrace is seeking an LPN/LVN to lead and coordinate nursing care in a long‑term care setting in Portland, OR. You will direct daily nursing assistant activities, ensure policy compliance, and participate in quality assurance efforts.

The role requires an active nursing license, CPR certification, and strong English communication skills. You will oversee care plans, charting, and collaboration with physicians to optimize resident outcomes.

Qualifications

• Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved LPN/LVN program.
• Current, unencumbered, active license to practice as an LPN/LVN in this state.
• Must be able to read, write, speak and understand English.
• Current CPR certification.

Responsibilities

• Direct the day-to-day functions of the nursing assistants.
• Ensure compliance with facility policies and procedures.
• Participate in developing and maintaining the nursing service quality assurance program.
• Assist in planning and coordinating resident care plans and discharge plans.
• Perform routine charting and documentation tasks.
• Lead nursing staff and conduct regular rounds.
• Respond to physician orders and ensure accurate transcription.
