Accela Rehab and Care at Springfield is seeking a licensed Practical Nurse (LPN) to join our skilled nursing and long-term care team. You will deliver high-quality nursing care, administer medications, and collaborate with RNs and CNAs in a warm, resident-focused environment.

Responsibilities of LPN:

• Deliver high-quality nursing care to residents in accordance with care plans

• Collaborate with RNs, CNAs, and interdisciplinary teams to ensure optimal outcomes

• Monitor residents' health, administer medications, and document care accurately

• Provide emotional support and build meaningful relationships with residents and their families

• Maintain accurate and detailed records of resident care and interactions
