Accela Rehab and Care Center at Springfield is seeking a dedicated LPN to provide high-quality nursing care in a skilled nursing and long-term care setting. You will work closely with RNs, CNAs, and the interdisciplinary team to implement care plans and monitor residents.

Qualifications

• Must be a PA-licensed LPN.
• Experience in skilled nursing or long-term care preferred.
• Excellent communication and teamwork.

Responsibilities

• Deliver high-quality nursing care to residents according to care plans.
• Collaborate with RNs, CNAs and interdisciplinary teams to ensure outcomes.
• Monitor health, administer medications, and document care accurately.
• Provide emotional support and build relationships with residents and families.
• Maintain accurate records of resident care and interactions.
