Latham Centers is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide and coordinate medical services for adults with complex special needs. This position plays an important role in supporting residents' health and well-being while working closely with a multidisciplinary team, families, medical providers, and program staff.

Location:Cape Cod, MA

Compensation:$40.40-42.87 per hour (based on experience)

Schedule:Monday-Friday 9:00 AM - 5:00 PM

What You'll Do:
  • As an LPN, you will help ensure residents receive high-quality, coordinated medical care while supporting their overall health and quality of life. Responsibilities include:
  • Monitor residents’ daily health needs and provide appropriate nursing support.
  • Coordinate preventive, specialized, and emergency medical appointments.
  • Coordinate and monitor medication ordering, administration, and documentation in accordance with MAPS and agency policies.
  • Monitor medical treatments and ensure appropriate follow-through by program staff.
  • Serve as a liaison between residents, families, referring agencies, medical providers, and community resources regarding medical needs.
  • Provide input regarding psychiatric consultations and medication reviews.
  • Maintain accurate medical records and assist with required program reports.
  • Participate in intake, discharge, and hospital discharge planning.
  • Maintain adequate medical supply inventory.
  • Participate in program development, implementation, and evaluation.
  • Provide direct care support when needed.
  • Participate in a rotating on-call schedule.
  • Work collaboratively with clinical staff, program teams, supervisors, families, and community providers.
What We're Looking For:
  • Current Massachusetts LPN license required
  • High school diploma or GED
  • Nursing degree from a community college or technical school
  • Valid driver’s license
  • One year of psychiatric or special needs experience preferred
  • Strong communication, organization, and time-management skills
  • Ability to work effectively as part of a multidisciplinary team
  • Compassionate, respectful, and professional approach when working with individuals with complex needs
  • Ability to maintain appropriate personal and professional boundaries
  • Ability to physically intervene when necessary to protect the safety of residents and staff
  • CPI (Crisis Prevention Intervention) – Will Train
  • CPR/First Aid – Will Train

Competitive Pay/Paid Time Off

Latham offers competitive pay with a $3 per hour weekend differential. Full time employees accrue 5 weeks of Earned Time Away in the first year of employment. $3/hour weekend differential.

Latham may be able to provide temporary individual housing based on availability.

Retirement Program

Latham provides up to a 5% employer match for any employee enrolled in the 403b retirement plan. Employees are eligible at the start of employment.

Childcare reimbursement is available to all benefited employees after 120 days of employment.

Paid Training and Tuition Reimbursement
