Job summary

The Gallatin County Rest Home is seeking an Admissions Coordinator who combines LPN clinical expertise with administrative duties to manage pre-admission assessments, admissions planning, and required documentation.

You will coordinate with nursing, social services, and the business office, help welcome new residents, and ensure regulatory compliance and census needs are met while providing compassionate service to residents and families.

Qualifications

• LPN license in good standing in Montana; CPR certification or ability to obtain within 6 months.
• Experience with resident admissions, assessments, and regulatory documentation preferred.
• Ability to coordinate care among nursing, social services, and other interdisciplinary teams.

Responsibilities

• Review referrals and medical records; conduct pre-admission assessments for placement and care needs.
• Coordinate admission dates, documentation, equipment, and services prior to admission.
• Serve as primary contact for residents and families; liaise with referral sources and departments.
• Maintain census, logs, and records; ensure compliance with federal and Montana requirements.
