Job summary

Gurwin Healthcare System in Commack, NY is seeking a full-time Clinical Assessor / Coordinator, LPN. You will review sub-acute and long-term care referrals from area hospitals, determine classifications, and guide appropriate admissions to Gurwin Jewish Health Care System.

The ideal candidate is a NY-licensed LPN with at least one year of hospital, long-term care, or CHHA experience. Monday–Friday, 9:00am–5:00pm; a $10,000 sign-on bonus is available along with comprehensive benefits and a

Qualifications

• Licensed LPN in New York State.
• Minimum 1 year experience in hospital, long-term care, or CHHA.
• Experience reviewing sub-acute and long-term care referrals.

Responsibilities

• Interpret PRI/RUG Classification System accurately.
• Manage referral caseload and respond via Navi/Allscripts and eFax.
• Assess classification and patient disposition upon screening.
• Coordinate referral via EMR and verify benefits.
• Coordinate bed board/census when needed.
• Assess potential patients at partnering hospitals.
• Communicate with residents, patients and families; engage the care team.
• Maintain knowledge of services available for new admissions.
