Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
• Graduation from an accredited practical nurse program.

Responsibilities

• Administer medications to clients via oral, injection, self-administration and other methods.
• Perform nursing assessments, including physical and mental health assessments and psychiatric screeners.
• Document medication administration and nursing assessments accurately and completely.
• Conduct MAR reviews and medication counts, monitor house stock and refill status.
• Communicate with clients regarding medication questions and needs, and stage refill requests to providers.
