Job summary

Vancrest of Urbana, Inc. Hillside is seeking a Licensed Practical Nurse (LPN) to implement a resident plan of care and deliver direct nursing services in our Ohio facility.

The LPN will administer treatments, assist in assessments and evaluations, and collaborate with the care team to ensure high-quality resident care. This role requires a current LPN license and supports professional growth within a team-focused environment.

Qualifications

• Must hold an active LPN license in Ohio.
• Experience in long-term care or resident care setting preferred.
• Ability to work 2nd or 3rd shift.

Responsibilities

• Implementing a resident plan of care.
• Performing direct nursing services such as administering treatments and assisting in assessments and evaluations
• Assisting with diagnostic and therapeutic procedures
• Delegating suitable components of the care plan to the nursing assistants, provides guidance and supervision when necessary
• Consulting with and validating data
• Recording pertinent information in the patient record
• Identifying and evaluating the resident's response to health problems and treatments
• Communicating residents care needs to others involved in the care
• Assuming responsibility for professional growth and development
