Job summary

Great Lakes Management is seeking an LPN for a Senior Living community in Golden Valley, MN. This full-time role offers competitive pay and benefits, including health care, 401(k) match, and paid time off. LPN certification in MN is required.

As part of the care team, you will oversee medications, maintain care plans, coordinate with physicians, staff, and families, and ensure adherence to state regulations. On-call rotation is possible and teamwork is essential.

Qualifications

• Active LPN license in Minnesota.
• Minimum 1 year experience in home health, assisted living, or long-term care preferred.
• Strong organizational skills with ability to work independently and initiate tasks.

Responsibilities

• Ensure residents meet admission criteria and monitor care plans.
• Maintain up-to-date resident charts and progress notes.
• Communicate changes in residents' needs to physicians, staff, and families.
• Coordinate resident services with other providers.
• Oversee medication administration and renewals with physician follow-up.
• Ensure proper medication procedures and report corrections to staff.
• Document information in resident logs for staff awareness.
• Provide supervisory visits in coordination with the RN.
• Collaborate with the care team to ensure safety and quality of care.
• Support emergency procedures and manage clinical scheduling.

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate .
• Works with manager to assure emergency procedures are in place
• Works with clinical leadership to manage the clinical schedule.
• Able to be in an on call rotation.

LPN Qualifications :

• Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
