Job summary

Qualifications

• Licensed Practical Nurse (LPN) license required.
• Compassionate, patient-focused and committed to quality care.
• Ability to work independently and with a team.
• Excellent communication and interpersonal skills.
• Maintain accurate records and handle sensitive information confidentially.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' health, vital signs, and response to care.
• Assist in developing and implementing individualized care plans.
• Provide nursing care including wound and catheter care; assist daily living activities.
• Collaborate with physicians, RNs, and healthcare team for coordinated care.
• Educate residents and families about health management.
• Ensure care complies with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when needed.
• Assist with training and supervising CNAs.
