Job summary

Countryside Manor in Fremont, OH is seeking compassionate Licensed Practical Nurses (LPNs) for Day Shift to join our clinical team in a skilled nursing and long-term care setting. We offer Day Shift with Full-Time, Part-Time and PRN options; every other weekend is required.

Qualifications

• Active, unencumbered Ohio LPN license.
• CPR certification current.
• Long-term care or skilled nursing experience preferred.

Responsibilities

• Provide direct resident care including medication administration, treatments, and nursing services.
• Monitor resident conditions and report changes to the clinical team.
• Supervise and support CNAs.
• Complete accurate clinical documentation.
• Collaborate with nurses, physicians, and the interdisciplinary team.
• Promote a safe, compassionate, resident-centered environment.
