Job summary

Countryside Manor in Fremont, OH is hiring Licensed Practical Nurses (LPNs) for Day Shift. Roles include direct patient care, medication administration, and collaboration with the care team in a skilled nursing setting.

Multiple schedules are available: Full-Time, Part-Time, and PRN, with every other weekend required. Benefits include medical, dental, vision, 401(k), PTO, and an $8,000 sign-on bonus for full-time LPNs.

Qualifications

• Active, unencumbered Ohio LPN license.
• Current CPR certification.
• Long-term care or skilled nursing experience preferred.
• New graduate LPNs are encouraged to apply.

Responsibilities

• Provide direct resident care including medication administration, treatments, and nursing services.
• Monitor resident conditions and promptly report changes to the appropriate clinical team.
• Supervise and support CNAs.
• Complete accurate and timely clinical documentation.
• Collaborate with nurses, physicians, and the interdisciplinary team to support resident care plans.
• Promote a safe, compassionate, and resident-centered environment.
