Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking an LPN for home health in Griffin, GA. The role focuses on compassionate, patient-centered care with a 1:1 approach in patients’ homes across Henry, Spalding, and Pike counties.

The LPN will provide medications and care per plan, document outcomes, and coordinate supplies while enabling independence and dignity for clients. Eligible candidates will hold an active LPN license, CPR, and a valid driver's license.

Qualifications

• LPN/LPT/LVN license with current unrestricted registration in the state of practice
• Valid driver’s license and proof of car insurance
• CPR professional certification
• 1+ years nursing experience preferred
• Home Health care experience preferred

Responsibilities

• Providing 1:1 care for client per plan of care while maintaining dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per plan of care
• Documenting nursing actions and communicating with physicians, case manager and family
• Coordinating supplies, medications and equipment to ensure availability
• Ensuring patient needs are met per physician instructions

Education

LPN/LPT/LVN Degree from an accredited college of nursing
