Job summary

Adoration Health in Douglas, GA is seeking a Licensed Practical Nurse for home health. The role focuses on delivering patient-centered, 1:1 care in a home setting. Schedule is Monday–Friday, 8:00am–5:00pm, with community-based coverage in Coffee and Atkinson Counties.

The LPN will administer medications, perform ostomy care, and document outcomes while coordinating with case managers and physicians. A valid GA LPN license and CPR are required.

Qualifications

• CPR certification is required.
• 1+ years of nursing experience preferred.
• Home Health care experience is preferred.
• Valid driver's license and auto insurance required.

Responsibilities

• Provide 1:1 patient care per plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate changes to physician and family.
• Coordinate supplies, medications, and equipment as needed.
• Ensure patient needs are met per physician instructions.
