Adoration Health, an affiliate of BrightSpring Health Services, is seeking a dedicated Home Health LPN to join our Griffin, GA team. You will provide 1:1 patient-centered care in the home, following the patient’s plan of care while maintaining dignity and independence.

You will collaborate with families, case managers, and physicians, document care, administer medications, and coordinate supplies. A valid GA LPN license, CPR certification, and a driver’s license are required; home health

Qualifications

• LPN/LPT/LVN license in state of practice and residence.
• CPR certification required.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsabilités

• Provide 1:1 home health nursing care following the patient’s plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate supplies, medications, and equipment to ensure availability.
• Ensure patient needs are met per physician instructions.
