Job summary

Always Compassionate Home Care in Ronkonkoma, NY is seeking dedicated Licensed Practical Nurses to join our pediatric and adult home care teams across Long Island. This part-time, per diem role offers flexible scheduling and the chance to provide compassionate care in clients' homes.

You will administer medications, monitor health, assist with daily living activities, and collaborate with families to implement personalized care plans. NY LPN license, BLS, and recent clearances are required.

Qualifications

• Current New York State LPN license.
• 6+ months of experience.
• BLS certification (American Heart Association).
• Physical exam within the past year.
• QuantiFERON within the past year.
• MMR Titers within the past ten years.

Responsibilities

• Provide direct patient care, including administering medications and treatments.
• Monitor and document patient health status and communicate changes promptly.
• Support clients with activities of daily living while maintaining their comfort and dignity.
• Collaborate with families and the care team to implement personalized care plans.
