Job summary

WeCare Centers in Pittsburgh, PA is seeking dedicated LPNs to join our team for full-time night shifts (11pm–7am). WeCare Centers provides essential nursing care, monitors patient health, administers medications, supports activities of daily living, and collaborates with the healthcare team to ensure quality resident care.

Qualifications

• Valid LPN license in the State of PA is required.

Responsibilities

• Conduct thorough assessments of patient health and status as part of admission and ongoing care.
• Provide regular patient status updates to the care team.
• Assist with implementing patient treatment plans under RN direction.
• Complete required documentation accurately and timely.
• Supervise, administer, and/or assist with medications.
• Provide direct care to residents as needed.
• Follow and contribute to the individualized plan of care for each resident.
• Report to the Director of Nursing and ensure residents' care standards are maintained.

We are looking for fulltime 11p-7am

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.

Qualifications

• Valid LPN license in the State of PA
