Job summary

WeCare at Heritage Care Center in Pittsburgh, PA is seeking dedicated LPNs to join our growing team for full-time night shifts from 11pm to 7am. This role focuses on compassionate, patient-centered care and collaboration with the health care team to maintain high standards for resident well-being.

As an LPN you will monitor patients, administer medications, assist with daily activities, complete documentation, and support plan of care under RN supervision.

Qualifications

• Valid LPN license in PA
• Full-time night shift 11pm–7am
• Commitment to high-quality resident care

Responsibilities

• Conduct assessments of patient health status and monitor changes
• Provide regular patient status updates
• Administer medications and supervise medication delivery as directed
• Assist residents with daily activities and plan of care under RN supervision
• Complete required documentation accurately and promptly
• Follow the plan of care for each resident and report to Nursing leadership

WeCare at Heritage Care Center, located at 5701 Phillips Avenue, Pittsburgh, PA, 15217 is actively seeking dedicated, LPNs to add to our growing team!

We are looking for fulltime 11p-7am

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.

Qualifications

• Valid LPN license in the State of PA
