Job summary

Bethany St. Joseph Corporation in Onalaska, WI is seeking an LPN/LVN for night and PM shifts at Onalaska Care Center. The role provides nursing care, medication administration, and assistance with care plans in a long-term care setting.

Licensed practical nurses in Wisconsin with CPR certification (or plan to obtain within 6 months) are welcome to apply. Part-time positions include various shift options and opportunities for training and growth.

Qualifications

• Active Wisconsin LPN/LVN license.
• CPR certification or ability to obtain within 6 months.
• Experience in Assisted Living or Long-Term Care is beneficial but not required.

Responsibilities

• Provide professional nursing care to residents.
• Verify physician orders and medication accuracy.
• Assist in developing resident care plans and conduct rounds.
• Prepare residents for admission, transfer or discharge and document changes.

Shift Availability

• Part-time, 16 Hours Per Pay period - NOC Shift
• Part-time, 56 Hours Per Pay Period -PM Shift

Job Responsibilities

• Provides professional nursing care to assigned residents and other areas as needed.
• Verifies Physician Orders and medication accuracy.
• Assists in developing resident care plans.
• Conducts routine rounds, noting and documenting any changes in resident condition (physical, emotional, social, spiritual) electronically.
• Prepares residents for admission, transfer, or discharge.
• Instructs residents and families in self-help/self-care.
• Supervises and directs assigned C.N.A.'s.
• Maintains resident rights and immediately reports suspected violations per policy.
• Administers medications and treatments as ordered by physicians.
• Assesses individual resident needs.
• Plans, implements, and evaluates care for a defined group of residents.

Qualifications and BenefitsRequirements:

• Must be currently licensed to practice in the state of Wisconsin as a Licensed Practical Nurse (LPN or LVN).
• Experience working in Assisted Living or Long-Term Care is beneficial, but not required. We will train the right fit for our team!
• Must be certified in C.P.R. or have the ability to obtain certification within 6 months.
