Job summary

Reliant Care Management Company in Oakwood, MO seeks an experienced LPN to provide compassionate nursing care, perform assessments, and coordinate care with the team.

You will supervise CNAs, coordinate with physicians, manage wound care, and respond to emergencies while ensuring compliance with facility policies.

Qualifications

• LPN licensure is required.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
