Job summary

Emerald Care Center Midwest in Midwest City, OK is seeking an LPN to join our nursing team. You will provide direct patient care, administer medications, and participate in care planning to improve resident outcomes.

This role includes 6am–6pm, 6pm–6am shifts or PRN opportunities. We value compassion, teamwork, and ongoing education for professional growth.

Qualifications

• Must be a licensed LPN in good standing.
• Experience in geriatric or long-term care preferred.
• Ability to perform assessments and care planning.
• Excellent communication with patients, families, and staff.

Responsibilities

• Provide direct patient care to residents, including administration of medications, wound care, and tracheostomy management
• Conduct vital signs, monitor for changes in condition, and report observations to nursing staff
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Participate in care conferences, rounds, and quality improvement initiatives to ensure seamless coordination of care
• Maintain accurate and detailed records of patient care and medication administration
• Collaborate with other healthcare professionals to ensure patient needs are met, and positive outcomes are achieved
