Job summary

Country Meadows Retirement Communities seeks a dedicated LPN to provide compassionate care in our assisted living communities. You will partner with residents, families, and physicians to implement care plans and ensure high-quality, around-the-clock nursing support.

Qualifications

• LPN license in the state where employed.
• Minimum of one-year experience in assisted living or other health care setting.
• Leadership skills and the ability to form relationships with residents, their loved ones and coworkers.
• Ability to perform the physical tasks required in this role.

Responsibilities

• Form bonds with residents, their loved ones and physicians to create care plans using critical thinking and good judgment.
• Coach co-workers to monitor and support residents’ needs with urgency and responsiveness.
• Round with physicians, oversee orders and monitor medications for quality care around the clock.
• Educate residents and families about health and wellness related to the care plan.
• Serve as and share responsibilities of Manager on Duty.

at $32.50/hour, based on shift preference, with the opportunity to earn more based on experience

Shift

3p-11p working 4 days in two weeks includes every other weekend and two weekdays
