Job summary

Country Meadows is seeking an experienced LPN to provide resident care in a home-like setting. You will use assessment skills and clinical leadership to impact care, build relationships with residents and families, and support the supervisory team.

Full-time and part-time shifts available, with structured schedules and off weekends/holidays on a rotating basis. Benefits include comprehensive coverage, 401(k) matching, paid time off, and on-campus meals.

Qualifications

• LPN license in the state where employed.
• Minimum of one-year experience in assisted living or health care setting.
• Leadership skills and the ability to form relationships with residents, loved ones and coworkers.
• Ability to perform the physical tasks required in this role.

Responsibilities

• Form bonds with residents, loved ones, and physicians to create care plans using critical thinking and good judgment.
• Coach coworkers to monitor and support resident needs with urgency and responsiveness.
• Round with physicians, oversee orders and monitor medications for quality care.
• Educate residents and families about health and wellness relevant to the care plan.
• Serve as and share responsibilities of Manager on Duty.

Pay starts at $32.50 - $35.50/hour, based on shift preference, with the opportunity to earn more based on experience

LPN Responsibilities:
