Job summary

Country Meadows in York, PA is seeking an LPN to deliver compassionate resident care with autonomy in a home-like setting. You will build relationships with residents and families, participate in care planning, and collaborate with the supervisory team to ensure high-quality care across shifts.

Qualifications

• LPN license in the state where employed.

Responsibilities

• Form bonds with residents, their loved ones and physicians to create care plans using critical thinking.
• Coach coworkers to monitor and support resident needs with urgency and responsiveness.
• Round with physicians, oversee orders and medications for quality care around the clock.
• Educate residents and families about health and wellness issues related to the care plan.
• Serve as and share the responsibilities of the Manager on Duty.

Pay starts at $32.50 - $34.50/hour, based on shift preference, with the opportunity to earn more based on experience.

LPN Responsibilities:
