Job summary

Keystone Educational Collaborative in Leominster, MA seeks a Program-based LPN to provide health services and support to students and staff. This full-time exempt role focuses on preventing barriers to learning through health interventions and coordinated care with families and providers.

Qualifications

• Massachusetts DPH license as an LPN required.
• High school diploma required.
• Driver’s license OR 7D license if operating school vehicles.
• At least 3 years of relevant experience preferred.
• Bachelor degree preferred.

Responsibilities

• Provide medical interventions, such as medication administration, care to ill children and first aid to injured students.
• Maintain appropriate medical records database.
• Work with medical providers and families to obtain and maintain student health information.
• Provide required medical records and information to district nurses.
• Work with the Program Supervisor or classroom staff to develop classroom and student safety plans.
• Assist in the development of student health care plans.
• Interface with family members and providers regarding student medical issues.
• Provide resources for and deliver health education to students and staff.
• Ensure confidentiality of student health related records and documents.
• Participate in team meetings, as appropriate.
• Ensure proper infection control procedures and equipment are in place.

Salary Schedule: Related Service Providers Salary Schedule* Introductory Period: 90 days

Reports To: School Nurse

Summary/Objective:

The Classroom/Program-based LPN provides health services to strengthen and facilitate the educational process by modifying or removing health-related barriers to learning in the individual students. Promote an optimal level of wellness of students and staff by providing preventative health services to facilitate the students and staff’s optimal physical, mental, emotional and social growth and development.

Essential Functions/Duties:

• Provide medical interventions, such as medication administration, care to ill children and first aid to injured students.
• Maintain appropriate medical records database.
• Work with medical providers and families and to obtain and maintain student health information.

Job Requirements

• Required Education and Experience:
  Massachusetts DPH License as a Licensed Practical Nurse.
  High School Degree.
  Valid Driver’s license OR 7D license if operating school vehicles.
• Preferred Education and Experience:
  Familiarity with Special Education settings and regulations.
  Familiarity with HIPAA and other health/privacy regulations.
  Ability to physically intervene with students in order to ensure safety.
• At least 3 year(s) of relevant experience preferred.
• Bachelor degree preferred.
• Citizenship, residency or work visa required
