Job summary

Luxor Healthcare is seeking an LPN to provide high-quality nursing care in a Missouri setting. You will assist residents with daily activities, document vitals and care changes, and work with RNs and physicians to implement care plans. A compassionate, patient-centered approach is essential.

Responsibilities include supporting ADLs, promoting engagement, and ensuring a safe environment while adhering to care plans and documentation standards. Missouri LPN license is required.

Qualifications

• Current LPN license in Missouri (Required).
• Strong communication, clinical, and organizational skills.
• Passion for providing person-centered care.
• Prior experience in long-term care, assisted living, rehab, or similar healthcare setting preferred.

• Deliver high-quality, compassionate nursing care to residents as per care plans.
• Support residents with activities of daily living (ADLs) respectfully.
• Promote resident engagement and socialization for mental and emotional well-being.
• Ensure a safe and secure environment for residents and staff.
• Follow the nursing process to assess, plan, implement, and evaluate individualized care plans.
• Document resident history, vitals, and clinical changes accurately.
• Communicate with RNs, physicians, and interdisciplinary team members.

Responsibilities of the LPN - Licensed Practical Nurse:

• Deliver high-quality, compassionate nursing care to residents in accordance with care plans.
• Support residents with activities of daily living (ADLs) in a respectful, dignified manner.
• Promote resident engagement and socialization to support mental and emotional well-being.
• Ensure a safe and secure environment for residents and staff.
• Follow the nursing process to assess, plan, implement, and evaluate individualized care plans.
• Accurately document resident history, vitals, and any clinical changes.
• Communicate effectively with registered nurses (RNs), physicians, and interdisciplinary team members.
