Job summary

Tennessee Quality Care is seeking a licensed practical nurse to join our home health team in Tennessee. You will provide direct patient care per the plan of care and support patients and families in self-care at home.

Responsibilities include accurate documentation, collaboration with RNs and other disciplines, and ongoing patient education to promote optimal recovery and comfort. A valid LPN/LVN license and CPR certification are required; reliable transportation is essential.

Qualifications

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has three years nursing experience; home health or medical-surgical experience preferred.
  • Licensed LPN/LVN in the state.
  • CPR certification required.

Responsibilities

  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services reporting needs to RN or Clinical Supervisor.
  • Uses equipment and supplies adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.

What Youll Do
  • Provides direct patient care as defined in the state Nurse Practice Act

  • Provides health care instructions to the patient as appropriate per assessment and plan of care
Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program
  • Has three 3 years nursing experience Communityhome health or medicalsurgical experience is preferred
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
  • Possess and maintain valid CPR certification
  • Must have reliable transportation current drivers license and appropriate automobile insurance
