Job summary

Genesis Healthcare is seeking a Social Services Specialist at our nursing center in Lancaster County to identify social, emotional, and mental health needs for patients/residents and coordinate services to support well-being and quality of life.

You will collaborate with the interdisciplinary team to develop care plans, educate staff on the social services role, assist with discharge planning, and maintain community referrals in accordance with NASW Code of Ethics and applicable regulations.

Qualifications

• Bachelor's degree in social work or human services required.
• State-specific licensure/certification as required for practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Education
