PACS is seeking a dedicated Dietitian to join our post-acutecare team in a long-term care setting. The role focuses on assessing nutritional needs, creating tailored care plans, and educating residents on healthy dietary choices. You will collaborate with a multidisciplinary team to deliver high-quality, patient-centered nutrition care.

Responsibilities include conducting assessments, developing care plans, monitoring progress, and providing ongoing nutrition education to residents and staff.

Qualifications

• Experience in dietetics and physiology.

• Educate patients on nutrition and meal planning.

Responsibilities

• Conduct comprehensive patient nutritional assessments.
• Develop, implement and monitor individualized nutrition care plans.
• Monitor progress and adjust meal plans as needed.
• Educate patients on proper nutrition, portion sizes, and meal timing.
• Collaborate with health care team to ensure comprehensive patient care.
• Provide nutrition expertise to the health care team.
• Review and revise care plans and assessments quarterly and annually.
