Job summary

PACS Group's Artesia Palms Care Center in Artesia, CA is seeking a dedicated Registered Dietitian for a part-time position. The role focuses on assessing residents' nutritional needs and developing tailored nutrition care plans within a long-term care setting.

You will educate residents on proper nutrition, monitor progress, and collaborate with nurses and physicians to adjust diets. Strong physiology and clinical nutrition knowledge are preferred to support individualized care.

Qualifications

• Dietitian with ability to assess nutritional needs and create care plans.
• Experience providing nutrition education to patients or residents.

Responsibilities

• Conduct comprehensive patient assessments to evaluate nutritional needs based on medical history and current health status.
• Develop, implement, and monitor individualized care plans that include dietary recommendations and interventions.
• Monitor patients' progress and adjust meal plans as needed
• Educate patients on proper nutrition, including food choices, portion sizes, and meal timing
• Collaborate with healthcare professionals to ensure comprehensive patient care
• Utilize physiology knowledge for effective dietary recommendations
• Perform diagnostic evaluations to assess patients' nutritional status
• Offer health coaching to support patients in achieving their dietary goals
• Develop preliminary and comprehensive assessments of the dietary needs of each resident.
• Provide nutrition expertise and serve as a resource to other members of the health care team.
• Participate in the planning, conducting, and scheduling of timely in-service training classes that ensure a well-educated dietary service department.
• Maintain current knowledge in clinical practices of nutritional therapies and treatment protocols.
• Ensure that all dietary personnel are aware of the Care Plan and that Care Plans are used in planning daily dietary services for the residents.
• Review nurses’ notes and meets with Direct Care Staff to determine if the Care Plan is being followed.
• Discuss problem areas with the Director of Nursing.
• Review and revise Care Plans and assessments as necessary, at least quarterly and annually.
