Qualifications

Responsibilities

• Perform PT evaluations (initial, quarterly, annual, as-needed).
• Provide therapeutic treatments and activities.
• Develop and monitor treatment plans focused on mobility, independence, and community living.
• Evaluate and recommend adaptive/assistive equipment.
• Participate in IDT meetings and care planning.
• Collaborate with medical providers, families, case managers, and staff.
• Educate staff on interventions, care plans, and equipment.
• Monitor progress and adjust treatment as needed.
• Maintain timely and accurate documentation.
• Collaborate with other allied health professionals.

We are seeking a LocumsPhysical Therapist (PT) in Lander, WY to provide therapy services to an adultresidential population, including individuals with traumatic brain injuries(TBI), organic brain injuries, dual diagnoses, and complex behavioral ormedical needs. The goal is to help residents improve functional skills,independence, and their ability to successfully transition to community-basedsettings.

PositionDetails

• Schedule: Monday–Friday
• Hours: Flexible based on provideravailability and facility needs
• Schedule Options: 5×8s, 4×10s, or 4.5×9s
• Coverage required across all fiveweekdays
• Adult patient population, ages 18+

Responsibilities

• Perform initial, quarterly, annual, andas-needed PT evaluations
• Provide therapeutic treatments andactivities
• Develop treatment plans focused onmobility, function, independence, and community living
• Evaluate and recommendadaptive/assistive equipment
• Participate in interdisciplinary team(IDT) meetings and care planning
• Collaborate with medical providers,families, case managers, and direct-care staff
• Train and educate staff on therapeuticinterventions, care plans, and equipment
• Monitor resident progress and adjusttreatment as needed
• Maintain timely and accuratedocumentation
• Collaborate with Occupational Therapy,Speech Therapy, Recreation, and other Allied Health professionals

Requirements

• Licensed Physical Therapist
• Active or eligible Wyoming PT license
• Bachelor's degree and completion of anaccredited Physical Therapy program
• BLS certification
