The position requires an Associate Degree and an active Texas PTA license, with Home Health experience preferred. Schedule runs Monday–Friday, 8:00 AM–4:30 PM, in the Dallas area with mileage reimbursement where applicable.

• Certification(s): Current state-required certifications and credentials
• Years of experience: Home Health experience preferred.
• Skills/Specialties/Technology: Home Health, Patient Care, Clinical Documentation, Care Planning, Travel Therapy, HCHB experience preferred

Responsibilities

• Provide physical therapy assistant services to patients in a home health setting under the supervision of a Physical Therapist.
• Implement and document treatment plans designed to improve patient mobility, strength, and functional independence.
• Travel to patient homes throughout the assigned DFW coverage area.
• Accurately complete clinical documentation and maintain patient records using HCHB or applicable EMR systems.
• Collaborate with interdisciplinary healthcare teams to ensure quality patient care and positive outcomes
