Job summary

FullShift Staffing, LLC is seeking an LVN/LPN to provide nursing care in a long‑term care setting in Modesto, California, ensuring patient needs are met in accordance with standards of practice, physician orders, and center policies.

Follows charting procedures, incident reporting, narcotic counts, and documentation of condition changes; utilizes the Quality Life Manual, notifies departments of changes, and rounds with physicians as needed. BLS/CPR certification preferred or willing to obtain.

Qualifications

• Long Term Care experience required.
• Proficient in PointClickCare charting.
• Current LVN/LPN license in practicing state (CA).
• BLS/CPR certification current or willingness to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient’s conditions including nutritional problems, infections, skin conditions, weight loss or other changes.
• Uses the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Documents and notifies applicable departments of patient room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

QUALIFICATIONS

• Current LVN/LPN license in practicing state.


• Current BLS/CPR certification or willingness to acquire one.


• Experience in nursing management is preferred but not required.
