Qualifications

• Current New Hampshire LNA or PCA certification preferred.
• Previous experience in home care, assisted living, or clinical caregiving environment.
• Valid driver’s license and reliable transportation.
• Successful completion of a background check and compliance with state requirements.

Responsibilities

• Personal & Hands-On Care: assist with daily routines including bathing, dressing, grooming and hygiene.
• Mobility & Safety: help clients transfer, ambulate, and prevent falls.
• Daily Living Support: prepare meals, light housekeeping, and organize living spaces.
• Companionship & Connection: build meaningful relationships with clients through conversation and activity.
• Health Observation & Reporting: track routines and report changes to care coordination team.
• Errands & Transportation: run local errands and provide transportation to appointments.
