Beacon Hospice, a part of Amedisys, seeks an LNA CNA Hospice Aide to deliver compassionate personal care at patients' homes across Seacoast NH & Maine. This role supports patients and families, adhering to care plans, safety standards, and teamwork with nurses.

Qualifications

• High school diploma or equivalent.
• Current state CNA certification.
• CNA experience within the last two years.
• Current CPR certification.
• Valid drivers license, reliable transportation and liability insurance.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to perform personal care tasks (ambulation, transfers, ROM, positioning, housekeeping).
• Plans and prepares nutritious meals, assists with feeding.
• Performs homemaking and environmental services as assigned.
• Observes, reports, and documents changes in patient status; refer to primary nurse as needed.
• Follow written care instructions provided by the registered nurse.
• Assists patients with prescribed exercises as directed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
