Job summary

Amedisys hospice care team in New Hampshire is seeking a compassionate LNA to join our home-based care. The role provides personal care, safety, meals, and support to patients in Nashua, Milford and surrounding areas, delivering care in the patient's home.

You will follow care plans, document changes, assist with exercises, observe status, and coordinate with nurses. Must have high school diploma, CNA license, CPR, and valid driver's license.

Qualifications

• High school diploma or equivalent.
• Current state certification as a nurse assistant per state requirements.
• CNA experience within the last two years and CPR certification.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and body mechanics during care and mobility tasks.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental services as assigned.
• Observes, reports, and documents changes in patient status to the nurse.
• Follows written care instructions and assists with prescribed exercises.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
