Job summary

Amedisys Home Health is seeking a Licensed Nursing Assistant to provide compassionate hospice care at patients' homes in the Concord area. You will assist with daily living activities, monitor patient status, and collaborate with nurses to implement the plan of care.

Qualifications

• High school diploma or equivalent is required.
• Current state CNA certification or registration is required.
• CNA experience within the last two years is preferred.
• Current CPR certification is required.

Responsibilities

• Provide personal care services to patients.
• Apply safety principles and proper body mechanics during caregiving tasks.
• Plan and prepare nutritious meals and assist with feeding.
• Perform homemaking and other environmental services as assigned.
• Observe, report, and document changes in patient condition to the nurse.
• Follow written instructions for personal care as outlined in the plan of care.
• Assist patients with prescribed exercises as instructed by a nurse or therapist.
• Perform and record basic urine tests for sugar, acetone, and albumin.
• Perform other duties as assigned.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
