Job summary

Jobot is seeking a Hospice Social Worker in the New York area. The role provides psychosocial support to Hospice and Palliative Care patients and their families in homecare settings, working as part of the IDT to coordinate care.

We offer a $15,000 sign-on bonus, 4 weeks vacation, 8 holidays, employer-paid Pension at 5.75%, 403B through Lincoln, medical/dental/vision, and generous tuition assistance and EAP programs. This is a dedicated role with a strong team and community focus.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of 3 years’ experience in Social Work in a healthcare setting.
• Community health, Hospice and Palliative Care or hospice experience preferred.

Responsibilities

• Provide psychosocial support counseling to Hospice and Palliative Care patients and families.
• Serve as the social work member of the IDT and document in EMR.
• Complete 20–25 visits per week and maintain case notes.
• Develop social work plan of care with goals and interventions.
• Make appropriate community referrals and transition planning.
• Ensure documentation with end dates and measurable outcomes.
