Job summary

Community Bridges, Inc. is seeking a Family Liaison who brings personal lived experience as a family member of someone receiving mental health or substance use services.

The role uses that experience to personalize care, help families understand services, and navigate referrals and recommendations from professionals. The Family Liaison will support clients and families across programs, collaborate with clinical teams, and assist in applying community resources to promote recovery and well-being.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Personal experience as a family member of someone receiving mental health/substance use services.
• Experience with children and families in a community-based program preferred.
• Health care related experience that provides exposure to behavioral and medical crisis situations preferred.

Responsibilities

• Assist clients and families in understanding services, referrals and applying recommendations from other professionals.
• Navigate current systems of care available in the community.
