Community Partners in Dover, NH seeks a Home Provider to offer a supportive home environment for an adult with developmental disabilities. The role involves living in a two-bedroom condo as part of a contractual arrangement and acting as a positive role model and companion.

The Home Providership includes oversight, and collaboration with a multidisciplinary team, with background checks required to ensure safety and quality of care.

Competenze

• Ability to live with and support an adult with developmental disabilities.
• Experience working with multidisciplinary teams.
• Willingness to rent and live in a two-bedroom condo.
• Background checks including criminal, OIG, BEAS and motor vehicle records.

Mansioni

• Provide a supportive home environment and model positive thinking.
• Assist with activities of daily living and community participation.
• Serve as a caregiver, mentor and positive role model for the individual.
