Job summary

Mosaic invites you to consider becoming a Home Provider in Arizona, where you welcome an individual with an intellectual disability into your home to share daily life and support their growth and independence.

You'll help with daily living skills, transportation, and community engagement, all while creating a safe, welcoming home and partnering with Mosaic's support team to ensure success for both you and the person you serve.

Qualifications

• Must be at least 21 years of age.
• Arizona resident due to state funding restrictions.
• Own or rent your home, apartment, condo, or townhouse.
• Active homeowner's or renter's insurance.
• Reliable transportation, valid driver's license, and current vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete required screenings, background checks, and training.
• Experience is preferred but not required.

Responsibilities

• Support daily living skills such as cooking, budgeting, cleaning, and decision-making.
• Assist with medication administration and medical appointments.
• Help individuals build connections, participate in their community, and experience a sense of belonging.
• Provide transportation and support with daily activities as needed.
• Maintain documentation and communication with Mosaic support team.
• Help someone build confidence, develop independence, and pursue personal goals.
