Tradition Homecare - NY in Brooklyn is seeking a compassionate Home Health Aide (HHA) for one-to-one patient care. You will assist with ADLs, monitor symptoms, and support patients in their homes, following detailed care plans.

Qualifications

• Completion of a state-approved Home Health Aide program.
• Minimum 1 year of paid home-health or hospice experience.
• Valid driver’s license and reliable transportation to reach clients’ homes.
• CPR certification (or willingness to obtain within 30 days).
• Clear, confident communication and a passion for relationship-centered care.

Responsibilities

• Hands-On ADL Support – Safely assist transfers, bathing, grooming, toileting, and mobility—protecting dignity.
• Home Environment & Infection Control – Change linens, light laundry, maintain a hazard-free, sanitized space.
• Medication & Symptom Monitoring – Provide reminders, observe changes, and document vitals or red-flag signs.
• Therapeutic Engagement – Read aloud, reminisce, guide ROM exercises, or accompany short walks for wellness.
• Accurate Documentation – Record care notes, progress, and concerns in our digital charting system.
