What You Will Do

You will help implement personalized care plans that support the whole person—mind, body, and spirit—and treat clients like family to enhance their quality of life.

Essential Functions

• Deliver exceptional care in clients' homes
• Follow personalized weekly schedules and care plans
• Perform daily activities safely (ADLs)
• Provide companionship and emotional support
• Assist with personal care, mobility, incontinence, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment
• Transport clients to appointments
• Document tasks and report changes in clients' condition
