Job summary

Mercy Health in Lima, OH seeks a Licensed Independent Social Worker (LISW) to provide advanced clinical social work services to patients, families, and groups across the continuum of care. The LISW conducts assessments, develops treatment plans, delivers psychotherapy, crisis intervention, and coordinates care with an interdisciplinary team.

This role requires an MSW, state LISW or LCSW licensure, and at least two years of direct practice.

Qualifications

• Master's degree in Social Work (MSW) required.
• Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) as required in state of practice.
• Minimum 2 years of social or human services experience including direct practice, assessment and treatment planning.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
• Provide evidence-based individual, group, and family therapy.
• Deliver crisis intervention, risk assessments, and safety planning as needed.
• Diagnose and treat mental health disorders within scope of practice.
• Coordinate referrals to community resources and higher levels of care.
• Document all patient encounters in accordance with standards.
